Referrals

There are no exclusion criteria; we welcome you to refer any patient whom you feel may benefit from our service.

If you would like to refer a patient please complete a referral form, which is available electronically as a template for Medical Director and Best Practice software. Templates can be downloaded from Primary Health Tasmania. Follow the instructions on how to download and import (PDF 361.6KB) these templates, or download the Blood Pressure Clinic Referral (PDF 564.8KB) form.

Please return completed referral forms to us via fax or post and we will contact the patient directly to arrange an appointment.

Automated Office Blood Pressure (AOBP)

Routine clinic blood pressure (BP) readings may give an inaccurate assessment of true BP level. Often the clinic BP is higher than home BP and 24 hour ambulatory BP readings[1]. It is now possible to obtain more accurate office BP readings by using programmable devices such as the Omron HEM-907 to measure AOBP while the patient rests quietly alone.
